Weight Loss Resistance: Could Insulin Resistance Be the Missing Piece?
You've cleaned up your diet, you're exercising consistently, and you're doing everything you've been told should lead to weight loss—but the scale refuses to budge.
If this sounds familiar, you're not alone.
While calorie intake and physical activity certainly influence body weight, they are only part of the picture. One of the most common and often overlooked reasons for weight loss resistance is insulin resistance.
Insulin resistance can develop years before blood sugar becomes abnormal, long before someone is diagnosed with prediabetes or type 2 diabetes. In fact, fasting insulin often begins rising 10–20 years before fasting glucose or HbA1c become elevated, making it one of the earliest warning signs of metabolic dysfunction (1).
The good news? Insulin resistance is often reversible, especially when identified early.
What Is Insulin?
Insulin is a hormone produced by the pancreas after you eat. Its primary role is to help move glucose (sugar) from your bloodstream into your cells, where it can be used for energy or stored for later (2).
You can think of insulin as a key that unlocks the door to your cells.
When everything is working properly:
You eat carbohydrates (or protein).
Blood sugar rises slightly.
The pancreas releases insulin.
Insulin helps glucose enter muscle, liver, and fat cells.
Blood sugar returns to normal.
Insulin also has several other important functions:
Promotes muscle building
Supports glycogen storage
Signals fullness after meals
Inhibits fat breakdown while energy is readily available
The problem arises when cells stop responding to insulin appropriately.
What Is Insulin Resistance?
Insulin resistance occurs when your body's cells become less responsive to insulin's signal.
Imagine someone knocking on your front door repeatedly. At first, you answer immediately. Eventually, if someone keeps knocking all day long, you begin ignoring it.
Your cells behave similarly.
As insulin levels remain elevated over time, muscle, liver, and fat cells become less responsive. To compensate, the pancreas produces even more insulin to achieve the same effect (3).
This creates a cycle:
Insulin resistance → higher insulin production → even greater insulin resistance
For years, blood sugar may remain completely normal because the pancreas is working overtime.
Eventually, however, the pancreas can no longer keep up.
Blood sugar begins to rise, followed by:
Prediabetes
Type 2 diabetes
Increased cardiovascular risk
Fatty liver disease
Polycystic ovary syndrome (PCOS)
Weight gain and difficulty losing weight
By the time fasting glucose becomes abnormal, insulin resistance has often been developing silently for many years (4).
Why Standard Blood Work Can Miss It
Many people are told their blood sugar is "normal," assuming everything is healthy metabolically.
Unfortunately, fasting glucose and HbA1c are late markers of insulin resistance.
The typical progression looks something like this:
Fasting insulin begins rising while glucose remains normal.
Insulin remains elevated. Blood sugar is still maintained within the normal range because the pancreas is producing extra insulin.
Blood sugar begins rising (prediabetes).
Type 2 diabetes develops as the pancreas can no longer compensate.
This is why I often include fasting insulin, along with fasting glucose, to calculate the Homeostatic Model Assessment of Insulin Resistance (HOMA-IR) when assessing patients with weight loss resistance, PCOS, fatty liver, metabolic syndrome, or a strong family history of diabetes (5).
Detecting insulin resistance early provides an opportunity to intervene long before blood sugar becomes abnormal.
Why Insulin Resistance Makes Weight Loss So Difficult
Many people assume weight gain causes insulin resistance.
In reality, the relationship works both ways.
Insulin resistance makes it much easier to gain weight and significantly harder to lose it.
When insulin levels remain chronically elevated (6):
Fat burning is suppressed.
More calories are directed toward fat storage.
Hunger and food cravings often increase.
Energy levels may decline, making exercise more difficult.
Blood sugar fluctuations can lead to fatigue and increased snacking.
This doesn't mean calories don't matter—but hormones determine how efficiently those calories are stored, used, and burned.
Trying to lose weight while insulin levels remain chronically elevated is a bit like trying to drive with the parking brake on.
Signs You May Have Insulin Resistance
Not everyone experiences symptoms, but common clues include:
Difficulty losing weight despite eating in a caloric deficit
Weight gain around the abdomen
Increased waist circumference
Fatigue after eating
Sugar or carbohydrate cravings
Afternoon energy crashes
Polycystic ovary syndrome (PCOS)
Fatty liver
Elevated triglycerides or low HDL cholesterol
Family history of type 2 diabetes
History of gestational diabetes
Skin tags or darkened skin folds (acanthosis nigricans)
The more of these factors present, the more likely insulin resistance is contributing.
The First Step towards addressing insulin resistance
Many people assume reversing insulin resistance requires an extreme diet.
In reality, sustainable habits often have the greatest impact.
These are the first four strategies I may recommend before considering more individualized nutrition plans or supplements.
1. Take a 10-Minute Walk After Meals
One of the simplest ways to improve blood sugar regulation is walking after eating.
When your muscles contract during walking, they can take up glucose from the bloodstream with less reliance on insulin. Even a short walk after meals has been shown to reduce post-meal glucose spikes and improve insulin sensitivity over time (7).
Aim for:
10 minutes after your largest meals
A comfortable pace
Consistency over intensity; build up to a 10-minute walk after each meal
2. Eat 30 Grams of Protein at Breakfast
Many breakfasts are heavily carbohydrate-based, leading to larger blood sugar spikes and hunger later in the day.
Including approximately 30 grams of protein at breakfast can help:
Improve satiety
Reduce mid-morning cravings
Support lean muscle mass
Reduce post-meal blood sugar elevations
Improve overall daily protein intake
Examples include Greek yogurt (if tolerated), cottage cheese, tofu scramble, protein smoothies, eggs with additional egg whites, or leftovers from dinner.
3. Aim for 30 Grams of Fibre Every Day
Fibre is one of the most powerful nutrients for improving metabolic health.
A higher fibre intake slows glucose absorption, nourishes beneficial gut bacteria, increases satiety, and has been associated with improved insulin sensitivity.
I recommend:
Working toward 30 grams or more of fibre daily
Tracking your intake using an app such as Cronometer for a week or two to understand your baseline
Increasing fibre gradually to minimize bloating or digestive discomfort
Drinking at least 2 litres of water daily as fibre intake increases
Excellent sources include vegetables, berries, legumes, chia seeds, flaxseed, oats, nuts, seeds, and whole grains where appropriate.
4. Eat in Alignment With Your Circadian Rhythm
Our metabolism follows a daily rhythm.
Eating late into the evening has been associated with poorer blood sugar regulation and reduced insulin sensitivity in many individuals (8).
Simple habits that can support metabolic health include:
Finishing your last meal 2–3 hours before bedtime
Avoiding late-night snacking when possible
Aiming for a 12-hour overnight fasting window (for example, finishing dinner at 7:00 p.m. and eating breakfast at 7:00 a.m.)
This overnight fasting period gives your body time away from digestion and may support improved insulin sensitivity and metabolic flexibility.
What Comes Next?
These four foundational habits are a great start towards stabilizing blood sugar and reversing insulin resistance.
Once these habits are well established, treatment becomes more individualized based on your health history, symptoms, laboratory findings, and goals.
Additional strategies may include:
Personalized nutrition plans based on your preferences and metabolic health
Resistance training and muscle-building programs
Stress management and sleep optimization
Targeted supplements to improve insulin sensitivity and correct nutritional deficiencies (e.g., magnesium, vitamin D)
Identifying and addressing contributing factors such as PCOS, chronic inflammation, menopause, medications, thyroid dysfunction, or gut health concerns
There is no one-size-fits-all approach. The most effective plan is one that addresses the root causes contributing to insulin resistance in your unique case.
The Bottom Line
Weight loss resistance is not simply a matter of willpower.
For many people, chronically elevated insulin is acting as a barrier to fat loss years before blood sugar tests become abnormal.
By identifying insulin resistance early and addressing it through targeted nutrition, movement, sleep, and lifestyle strategies, we can often improve metabolic health long before prediabetes develops.
